* Adds buildAll/cleanAll scripts and a per-driver solution.
* Makes all binaries drop to ivshmem\Install.
* Renames test.exe to ivshmem-test.exe.
* Fixes SDV build (it didn't like the <OutDir> and <IntDir> settings
  for some reason).

Solution files should be tracked like any other files.
Also fixing a few more .gitignore entries.

This commit eliminates 2012-defines.h which was being generated
during build. The required version macros are now defined in
DriverVersion.props included from the project.

The project is also fully built with VS now.
